How to apply to the Disabled Students' Residence Program

Please review the DSRP eligibility requirements before starting the application process. Note: you will need the free Adobe Reader software in order to view the pdf forms. If you need assistance with the forms, or the application process, please contact DSRP staff.

Application Process

1. You must sign up with the California Department of Rehabilitation (DOR) before you can apply to the Residence Program.
2. Download and fill out the DSRP forms:
   
   • Information Please Form [pdf]
   • Release of Information Form [pdf]
   • DSRP is Not a Medical Facility Form [pdf]
3. Download and fill out the University Health Center (Tang) form:
   
   • Patient Information Form
4. Submit all four completed forms to:
