Proctoring--Faculty Request for Exam Accommodations

The online request form for Fall 2019 will open on Monday, August 26th

The Berkeley campus uses a shared model for testing accommodations and proctoring services. While Academic Departments and Faculty are primarily responsible for providing testing accommodations to students with disabilities, Proctoring Services provides support (proctors and space) on a first-come, first-served basis to Departments and Faculty upon request.

 

Space is limited and proctoring services are not guaranteed. We make an extra effort to provide proctoring services for complex accommodations such as room alone, scribe, 200% (or more) time, use of a computer or other assistive technology.

Note: DSP will not proctor any exams during RRR week in alignment with campus policy. 

DSP, in collaboration with Academic Departments and Faculty, seeks to ensure an equitable and sustainable testing environment.

To request DSP proctoring services please fill out the online request form.


  • The online request form should be filled out at least 2 weeks before your first exam. 

  • Please fill out ONE online form for each Course Control Number (CCN). Include quizzes, midterms, and finals in this request.

  • For cross-listed classes, you must submit a separate online request form for each CCN.

  • If the student requires a scribe we need at least five (5) business days from the date of your online request to schedule scribes.
